| Referee Evaluation Form | For use with U11 - U19 games To assist in the development of the game of soccer it is essential that all parties be assessed in their involvement to the organization. Team Officials are asked to complete this form (at the conclusion of the game) to provide constructive criticism or positive remarks based on an referee’s in-game performance. Specifically the referee’s professionalism, knowledge of the rules, effort, movement on the pitch, clear signals, consistency and control of the game.
